A few helpful answers

Before you get started.

Can a family member or professional make a referral?

Yes. You can prepare a referral as a family member, support coordinator or professional. Please ensure you have the participant’s consent or appropriate authority to share their information.

What information should I include?

Start with your contact details, the type of support you’re seeking and a brief overview of your goals. Detailed reports and NDIS information can be discussed through an agreed channel later.

How do I know if my NDIS funding can be used?

Funding arrangements need to be checked for the specific service and your plan management type before services are agreed. Include this question in your enquiry so the options can be discussed with you.

How is my enquiry or referral sent?

The form sends your entries through FormSubmit to gracepointalliedhealth@hotmail.com. Please share only initial contact details and a brief overview. Do not include medical reports, NDIS numbers or sensitive health information. You can also contact GracePoint by phone or email.
